If you are are already seeing another provider for psychiatry medications but would like to see us just for Spravato, please note that you must discuss this with your provider and ask them to fax a referral form to our office. If you are not already seeing another provider and if you would like to become a new patient and are interested in Spravato, please note that you must currently already be taking an antidepressant medication and have also tried and had insufficient results from another antidepressant in the past. If you are not currently seeing a psychiatric provider and are interested in Spravato and also need a prescriber to assist you in becoming eligible. Please fill out the following general new patient paperwork and we will work on getting you in to see a provider as soon as possible.
