Please contact the office if the new patient is under the age of 18 prior to filling out the paperwork. Please note that our paperwork is lengthy. This allows your provider to have an integrative mental health plan ready for you prior to your first visit with them. If you have difficulty filling out paperwork, please let the office know as well, assistance may be available.
